Mais derri\u00e8re une consultation ou une op\u00e9ration chirurgicale, se cache une organisation complexe\u2026 souvent invisible, mais essentielle pour am\u00e9liorer l\u2019exp\u00e9rience du patient. Pour mieux la comprendre \u2013 et surtout l\u2019am\u00e9liorer \u2013 une discipline discr\u00e8te, mais puissante entre en sc\u00e8ne : <strong>la mod\u00e9lisation math\u00e9matique<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>C\u2019est quoi la mod\u00e9lisation math\u00e9matique ?<\/strong><\/h2>\n\n\n\n<p>La mod\u00e9lisation math\u00e9matique, c\u2019est un peu comme faire une carte tr\u00e8s d\u00e9taill\u00e9e d\u2019un syst\u00e8me, ici l\u2019h\u00f4pital, en repr\u00e9sentant toutes ses composantes : les t\u00e2ches, les informations \u00e9chang\u00e9es, les ressources humaines et mat\u00e9rielles, et l\u2019organisation globale.<\/p>\n\n\n\n<p>La mod\u00e9lisation formelle s\u2019appuie sur la <em>syst\u00e9mique<\/em>, une approche qui consid\u00e8re un h\u00f4pital non pas comme une somme d\u2019\u00e9l\u00e9ments isol\u00e9s, mais comme un syst\u00e8me global, o\u00f9 chaque action, chaque acteur et chaque ressource interagit avec les autres. Un retard \u00e0 l\u2019accueil peut impacter la consultation, qui elle-m\u00eame influence un rendez-vous pour obtenir un scanner, jusqu\u2019\u00e0 la sortie de l\u2019h\u00f4pital. En pensant l\u2019h\u00f4pital comme un ensemble de processus interconnect\u00e9s, on peut mieux comprendre ses dysfonctionnements, anticiper les effets en cha\u00eene, et imaginer des am\u00e9liorations durables.<\/p>\n\n\n\n<p>La figure ci-dessous pr\u00e9sente une approche de mod\u00e9lisation de l&rsquo;architecture d&rsquo;un syst\u00e8me, dans lequel trois sous-syst\u00e8mes sont en interaction : le syst\u00e8me op\u00e9rationnel regroupe l&rsquo;ensemble des processus du syst\u00e8me (la prise en charge d&rsquo;un patient par exemple), le syst\u00e8me de pilotage regroupe les m\u00e9canismes de prise de d\u00e9cision (quel diagnostic pour l&rsquo;orientation d&rsquo;un patient, ou encore quelles ressources affecter \u00e0 un service de soins), et le syst\u00e8me d&rsquo;information regroupe l&rsquo;ensemble des donn\u00e9es n\u00e9cessaires pour la mise en \u0153uvre du syst\u00e8me et la prise de d\u00e9cision.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"620\" height=\"299\" src=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-1.png\" alt=\"\" class=\"wp-image-171\" srcset=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-1.png 620w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-1-300x145.png 300w\" sizes=\"auto, (max-width: 620px) 100vw, 620px\" \/><\/figure>\n\n\n\n<p>Imaginez que vous puissiez simuler un service de consultation, pr\u00e9voir ce qui se passerait si vous ajoutiez un poste d\u2019infirmier ou changiez l\u2019ordre des t\u00e2ches. Gr\u00e2ce \u00e0 la mod\u00e9lisation math\u00e9matique coupl\u00e9e \u00e0 d\u2019autres outils comme la simulation, c\u2019est possible.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Pourquoi est-ce utile en sant\u00e9 ?<\/strong><\/h2>\n\n\n\n<p>Un h\u00f4pital (une clinique, un centre de lutte contre le cancer, un EPHAD \u2026), c\u2019est un peu comme une ville miniature : on y soigne, on y planifie, on y d\u00e9cide, on y communique. Mais ces interactions sont souvent complexes, et les erreurs ou les lenteurs peuvent avoir un impact direct sur les soins.<\/p>\n\n\n\n<p>Gr\u00e2ce \u00e0 la mod\u00e9lisation :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>On peut proposer <strong>un formalisme clair et sans zones d&rsquo;ombre<\/strong> pour communiquer entre partenaires d&rsquo;un projet et appliquer ensuite plusieurs outils d&rsquo;analyse.<\/li>\n\n\n\n<li>On peut <strong>tester diff\u00e9rents sc\u00e9narios<\/strong> sans risque, comme dans un simulateur.<\/li>\n\n\n\n<li>On peut <strong>anticiper les engorgements<\/strong> et am\u00e9liorer les temps d\u2019attente.<\/li>\n\n\n\n<li>On peut <strong>\u00e9valuer les co\u00fbts r\u00e9els<\/strong> d\u2019une proc\u00e9dure, jusqu\u2019au niveau micro.<\/li>\n\n\n\n<li>On peut proposer des m\u00e9thodes pour <strong>communiquer efficacement<\/strong> entre m\u00e9decins, gestionnaires et informaticiens.<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1024\" src=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6.png\" alt=\"\" class=\"wp-image-173\" srcset=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6.png 1024w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6-300x300.png 300w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6-150x150.png 150w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6-768x768.png 768w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/a0b40440-faf9-40e0-9d2f-34a4e2489ee6-100x100.png 100w\" sizes=\"auto, (max-width: 767px) 89vw, (max-width: 1000px) 54vw, (max-width: 1071px) 543px, 580px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Quels outils sont utilis\u00e9s ?<\/strong><\/h2>\n\n\n\n<p>Plusieurs m\u00e9thodes ont \u00e9t\u00e9 d\u00e9velopp\u00e9es au fil du temps pour mod\u00e9liser des syst\u00e8mes complexes comme les h\u00f4pitaux. Parmi les pionniers de cette approche figure le professeur Fran\u00e7ois Vernadat, qui a largement contribu\u00e9 dans les ann\u00e9es 1990 \u00e0 formaliser les techniques de mod\u00e9lisation d\u2019entreprise (F. Vernadat, Techniques de Mod\u00e9lisation en Entreprise : Applications aux Processus Op\u00e9rationnels, Economica, Paris, 1999). Ces travaux s\u2019inscrivent dans une discipline largement utilis\u00e9e dans l\u2019industrie : le g\u00e9nie industriel.&nbsp;<\/p>\n\n\n\n<p>Plusieurs m\u00e9thodes existent. Parmi les plus connues :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>IDEF0<\/strong> : une m\u00e9thode graphique d\u00e9velopp\u00e9e dans les ann\u00e9es 1970, utilis\u00e9e pour d\u00e9composer les fonctions d\u2019un syst\u00e8me en blocs clairs et interconnect\u00e9s.<br><strong>ARIS<\/strong> : un cadre m\u00e9thodologique complet apparu dans les ann\u00e9es 1990, qui permet de repr\u00e9senter diff\u00e9rentes vues d\u2019un syst\u00e8me (fonctionnelle, informationnelle, organisationnelle, etc.).<\/li>\n\n\n\n<li><strong>UML<\/strong> (<em>Unified Modeling Language<\/em>) : un langage de mod\u00e9lisation issu du g\u00e9nie logiciel, qui permet de repr\u00e9senter les objets, les interactions et les comportements d\u2019un syst\u00e8me. Tr\u00e8s utilis\u00e9 pour concevoir les logiciels hospitaliers ou mod\u00e9liser des syst\u00e8mes d\u2019information de sant\u00e9.<\/li>\n\n\n\n<li><strong>BPMN<\/strong> (<em>Business Process Model and Notation<\/em>) : un langage graphique con\u00e7u pour d\u00e9crire les processus m\u00e9tiers de mani\u00e8re simple et intuitive. Il est particuli\u00e8rement adapt\u00e9 \u00e0 la cartographie des parcours patients et \u00e0 la collaboration entre \u00e9quipes m\u00e9dicales et administratives.<\/li>\n<\/ul>\n\n\n\n<p>La figure ci-dessous illustre la brique de base d&rsquo;un mod\u00e8le IDEF0 : une fonction (une activit\u00e9) transforme un input en un output sous certaines contraintes (<em>control<\/em>) et gr\u00e2ce \u00e0 certains m\u00e9canismes (<em>mechanism <\/em>ou ressources).<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"511\" height=\"272\" src=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-2.png\" alt=\"\" class=\"wp-image-172\" srcset=\"https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-2.png 511w, https:\/\/dali.science\/wp-content\/uploads\/2025\/05\/image-2-300x160.png 300w\" sizes=\"auto, (max-width: 511px) 100vw, 511px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Un exemple concret : l\u2019hospitalisation d\u2019une personne \u00e2g\u00e9e suite \u00e0 un passage aux urgences<\/strong><\/h2>\n\n\n\n<p>Prenons le parcours \u00e0 l\u2019h\u00f4pital d\u2019une personne \u00e2g\u00e9e : passage aux urgences, hospitalisation en court s\u00e9jour g\u00e9riatrique (CSG), soins m\u00e9dicaux et de r\u00e9adaptation (SMR). Derri\u00e8re ce sc\u00e9nario en apparence banal se cachent plusieurs activit\u00e9s, ressources (logiciels, personnels, documents), d\u00e9cisions \u00e0 prendre. Gr\u00e2ce \u00e0 la mod\u00e9lisation, on peut analyser chaque \u00e9tape, rep\u00e9rer les goulots d\u2019\u00e9tranglement pouvant g\u00e9n\u00e9rer de l\u2019attente pour le patient, calculer les co\u00fbts, et m\u00eame proposer une nouvelle organisation plus fluide.<\/p>\n\n\n\n<p>La figure ci-dessous pr\u00e9sente un logigramme simplifi\u00e9 de ce processus : les patients arrivent dans le service d\u2019urgence. Si une hospitalisation est n\u00e9cessaire, le patient est transf\u00e9r\u00e9 vers un service de court s\u00e9jour g\u00e9riatrique, ou un autre service selon la pathologie. Suite au court s\u00e9jour g\u00e9riatrique, si le patient n\u00e9cessite un suivi, il peut \u00eatre transf\u00e9r\u00e9 vers un service de soins m\u00e9dicaux et de r\u00e9adaptation (SMR).<\/p>\n\n\n\n<p>Dans ce parcours, plusieurs goulots d\u2019\u00e9tranglement peuvent exister \u00e0 chaque \u00e9tape : entre les urgences et le CSG, entre le CSG, et le SMR, etc. Ces goulots peuvent \u00eatre dus \u00e0 un manque de ressources (on parle de lits associ\u00e9s aux ressources humaines ad\u00e9quates), et d\u00e9gradent la qualit\u00e9 de service. Ainsi, un indicateur important que nous pouvons mesurer est la dur\u00e9e de s\u00e9jour dans les diff\u00e9rents services. Un autre indicateur important pourrait \u00eatre le taux d\u2019occupation des lits.<\/p>\n\n\n\n<p>Afin d\u2019\u00e9valuer les performances de ce syst\u00e8me, il est n\u00e9cessaire d\u2019impl\u00e9menter le mod\u00e8le dans un outil de simulation \u00e0 \u00e9v\u00e9nements discrets par exemple. Cette impl\u00e9mentation requiert aussi des donn\u00e9es \u00e0 collecter dans l\u2019h\u00f4pital.
